Accounting and Tax for GPs, Specialists and Medical Practice Owners

Medicine Is Complicated Enough. Your Tax Should Not Be.

You look after your patients. We look after the tax, the structures and the paperwork, so nothing catches you out at the end of the financial year.

Most doctors we meet are not short on income. They are short on time. Somewhere between clinics, hospital shifts and on call rosters, the money side ends up in a shoebox. If any of these sound familiar, you are in the right place:

  • Income coming from a practice, a hospital and maybe a private room, all taxed differently
  • Service and facility fees you pay to a practice, and no clear picture of what they mean for your tax
  • A company or trust someone set up years ago, and you are not sure it still suits you
  • Running a practice, with staff, contractors, payroll, BAS and payroll tax all landing on your desk

Think of us as the pre op checklist for your finances. Everything is checked before it matters, so there are no surprises later.

How We Help GPs, Specialists and Practice Owners

For GPs and specialists

  • Tax returns done properly. We bring together income from every practice, hospital and private source, and claim the deductions you are entitled to, such as professional indemnity, registration fees and professional development where they relate to earning your income. See our tax return services.
  • Personal services income. We check how the personal services income rules apply to you, so your income is reported the right way the first time.
  • Structure reviews. If you earn through a company or trust, we review whether it still makes sense for you and whether it lines up with current ATO guidance.
  • Contractor arrangements. If you work as a contractor in someone else’s practice, we explain the tax side of your service fee arrangement in plain English, and work alongside your lawyer where the agreement needs legal advice.

For medical practice owners

  • Bookkeeping, BAS and payroll. GST, PAYG and Single Touch Payroll handled and lodged on time, so you can stay focused on patients. See our bookkeeping services.
  • Payroll tax reviews. We check which payments in your practice count towards Queensland payroll tax and which do not, so you are neither overpaying nor exposed.
  • Financial statements and business tax. Clear year end accounts and a business tax return that reflects how your practice really runs.
  • Restructuring and planning. Bringing in a partner, buying in, or planning your exit. Our restructuring and business lifecycle planning services help you make the move with the tax side sorted first.

What Has Changed for Doctors Recently

The ATO is looking closely at how professionals use companies and trusts

The ATO has published guidance, PCG 2025/5, on personal services businesses and the general anti avoidance rules. In short, a structure that mainly shifts income to family members on lower tax rates is more likely to be questioned. Doctors are one of the professions the guidance gives as an example. If you use a company or trust, it is worth having it reviewed against this guidance.

Queensland payroll tax for GPs

Since 1 December 2024, wages a Queensland medical practice pays to GPs, whether they are contractors or employees, have been exempt from payroll tax. The exemption covers GPs only. Payments to other specialists, allied health practitioners and practice staff can still count towards payroll tax, so practice owners should check where they stand.

Super and self managed super funds

We are not financial planners or investment advisers, but we can partner with your adviser to complete the compliance requirements for your fund. We make sure it only pays as much tax as it has to, and that all your lodgements are up to date and on time. Learn more about our self managed super fund services.

How It Works

  1. Book a meeting. In person in Helensvale or online, at a time that suits your roster.
  2. We review your setup. Your recent returns, your structures and how your income is earned.
  3. You get a clear plan. What is working, what needs fixing, and what it will cost, in plain English.
  4. We look after it all year. Lodgements, deadlines and questions handled, so tax time is a non event.

Questions Doctors Ask Us

Yes. We offer online consultations, so we can work with doctors and practices anywhere in Australia.

It depends on how you work. The personal services income rules and the ATO’s guidance in PCG 2025/5 limit what a structure can do for most individual practitioners. We review your situation against those rules before recommending any change.

Possibly. Wages paid to GPs have been exempt since 1 December 2024, but payments to other doctors, allied health practitioners and staff can still count. Whether you need to pay depends on your total wages, so it is worth a review.

Yes, with the tax, compliance and lodgement side. We are not financial planners, so we work alongside your licensed adviser for investment and contribution advice.
